1. Download the appropriate Information Sheet (State Council, District Deputy or Local Council) to a Word doc.

2. Enter the appropriate State Position or State Council Job Title, OR

District Number, OR

Council Number.

3. Enter your name OR, when applicable, the Grand Knight’s or Financial Secretary’s name.

4. Enter the correction(s) ONLY to be placed in the State Directory.

5. Save the corrected downloaded file.

6. E-MAIL the saved downloaded file to the CSD Editor, Steve Mieczkowski, FDD at R73X@hotmail.com as an attachment.

Print the saved corrected downloaded file and mail to:
